Sore Breasts While Breastfeeding? It Might Be More Than Just a Blocked Duct

Breast pain during breastfeeding can be stressful and uncomfortable, especially when you are already recovering after birth and caring for a newborn. Many women experience blocked ducts, breast tenderness or mastitis during the postnatal period, but early support can often help symptoms settle faster.
You may notice:
A painful or tender area in the breast
Swelling, firmness or redness
Pain while feeding
Reduced milk flow
Fever or flu-like symptoms in more severe cases
What was traditionally called a “blocked duct” is now understood to often involve inflammation and swelling within the breast tissue, affecting milk flow and causing discomfort.

What are blocked ducts and Why do they happen?
When milk does not drain completely from the breasts, it can cause a blockage in the milk duct. This can cause a lump to form, as well as swelling and inflammation. This is common during the first weeks of breastfeeding, but it can also occur when lactation has been disrupted by skipped feedings, a poor latch, tight clothing or stress. In the absence of early treatment, blocked ducts may develop into mastitis. This inflammatory condition can cause fever and flu-like symptoms as well as more intense breast discomfort. To avoid complications and relieve discomfort, it is important to receive prompt treatment.
What Can Physiotherapy Do for You?
In the past, women were often advised to use firm massage to “clear the blockage.” However, newer guidelines suggest that aggressive massage may actually worsen inflammation and irritate the tissue further.
Physiotherapy for mastitis and blocked ducts focuses on gentle, evidence-based treatment to help reduce pain and support recovery. Treatment may include therapeutic ultrasound, gentle manual therapy, postural advice and strategies to improve feeding comfort and milk flow. Early treatment for blocked ducts and mastitis may help reduce symptoms and prevent progression.
If you are experiencing ongoing breast pain while breastfeeding, recurrent blocked ducts or mastitis symptoms, women’s health physiotherapy may help support a more comfortable recovery.
